Eligibility
All children born to Pakistani citizens or in Pakistan. Should be registered within 30 days of birth for free registration; late registration incurs a fee.

- • Birth notification from hospital/midwife
- • Original CNIC of mother
- • Original CNIC of father (if alive)
- • Marriage certificate (certified copy)
- • For late registration: B-Form application form and fee
